The Southwest Iowa Mental Health and Disability Services Region (SWIA MHDS) serves a nine-county area in the beautiful rolling hills of the most southwest corner of Iowa. The geographic area includes the counties of Cass, Fremont, Harrison, Mills, Monona, Montgomery, Page, Pottawattamie and Shelby. The region’s plan is designed to improve health, hope and successful outcomes for children with behavioral health needs as well as adults who have mental health or intellectual/developmental disabilities, including those with multi-occurring substance use issues, health issues, physical disabilities, brain injuries and other complex human service needs. 

Coordination of Services — SWIA MHDS professionals assist individuals in accessing resources they need to improve hope, health and successful outcomes.

Financial Support — SWIA MHDS professionals help individuals navigate through the process of applying for and securing necessary financial support for their immediate needs.

Resources

Locations — The main office for the Southwest Iowa MHDS Region is located at 227 S. 6th St. Suite 128 in Council Bluffs and can be reached at 712-328-5645. For information and referral, you may contact the Service Intake Coordinator directly at 712-328-5812.

Provider Access Points —  SWIA MHDS offers several provider access points.  Providers at these locations are trained to help individuals fill out the region application and can serve as your first point of contact with the region.

Resource Connection — SWIA MHDS maintains a large directory of resources available within the region.

Applications — Individuals interested in seeking funding must complete an application. You may visit or call a local office for the form, or access the downloadable form on our application page. Once completed, the application should be returned to the main office at 227 S. 6th St., Suite 128, Council Bluffs, IA 51501.
